A merit-based scholarship will be presented each year to a deserving Ford City High School student who wishes to pursue a post secondary education in the field of nursing in loving memory of Helen W. Lazaroff, former resident of Ford City.

This scholarship will be apportioned according to course load: full-time (minimum of 12 hours), ¾-time (nine hours), or half-time (six hours).  Scholarships may also be granted to students taking professional development (non-credit) courses.  The scholarship committee reserves the right to withhold the award of any scholarship for reasons deemed appropriate by the committee.    

Eligibility Criteria

• Ford City High School graduate
• Declared student in a degree-granting program in the field of nursing
• Attendance at an institution of higher learning in the great state of Pennsylvania
• United States Citizen
• High School GPA of 3.0 or above
• College GPA of 3.0 or above

Submittals

• Nominations must be typed or computer-generated using the Scholarship Nomination Form specifically designed for this purpose.  A transmittal letter explaining why applicant chose or is choosing a career in nursing with a list of extracurricular and community activities is also required.
• Three letters of recommendation must be included with applications for degree-granting programs.
• A letter of request indicating professional development plans listing course(s) with costs (registration, fees, books, etc.) must be included for non-credit courses.  Please note that letters of recommendation, ACT/SAT scores, and GPA are not required for professional development.
• Applications must be received by close of business on 30 April of each year for the subsequent fall term at the following address:
